THE ORGANIZATION
Asian Pacific Community Counseling (APCC) promotes mental health and wellness in Asian and Pacific Islander communities through culturally and linguistically relevant outreach, prevention, education, counseling, and recovery support services and by providing training and consultation.
Under the general direction of the Clinical Manager/Supervisor and Quality Improvement Manager, the Peer Specialist will work as part of the multi-disciplinary team and will assist in providing and coordinating services to clients who are referred to TWC. The Peer Specialist is responsible for giving and receiving support through a mutual exchange of experiences, hope, and strengths to assist clients in obtaining the services of their choice. This position is responsible for linking the client and family member to community resources internal and external to the mental health system, to assists in helping the client learn coping skills, and to assist the client in developing his/her wellness and recovery action plan congruent with cultural norms and expectations. The Peer Specialist will work in a variety of settings other than the office, including the client’s environment or other community settings.

Essential Duties
- Collaborate with clinicians, counselors, specialists, and psychiatrists in coordinating and providing services to clients.
- Assist in linking the client and family member with resources in the community per the client plan.
- Give and receive support through a mutual exchange of experiences, strengths, and hope to assist the client in obtaining the services needed.
- Assist in helping the client learn effective coping skills.
- Assist the client in developing and following a wellness and recovery plan unique to their needs and desires.
- Meet minimum billable minutes per month to meet Sacramento County requirements.
- Enter and complete required documentation in Electronic Health Record within the standard 3 business days of providing client care to meet county requirements.
- Provide advocacy for clients in navigating the resources and services in the mental health system and other systems as part of their recovery.
- Meet with the client in their home or community setting to provide advocacy and services.
- Assist in the evaluation process of clients in referrals to other programs and services.
- Participate in staff meetings and training.
- Participate in case conferences and case reviews as necessary.
- Attend meetings at the agency or in the community as needed and required.
- Participate in professional skill development as necessary to perform job duties.
- Complete documentation as required.
- Assist in building and maintaining a list of community resources for clients and family members.
- Provide a positive environment for clients, family members, and other staff members.
- Make successful links with community agencies for resources, information, and referrals.
Knowledge, Skills, and Abilities
- Must have life experience as a client or a family member of mental health services with mental illness.
- Ability to work effectively and competently with staff, clients, family members and other providers who represent the diversity of the community.
- Ability to deal with conflict, crises, and problem situations.
- Ability to communicate effectively, orally and in writing, with program staff, clients and family members, and other agencies in the community.
- Ability to work as a member of a team.
- Ability to pass Peer Certification training.
- Demonstrated ability with Basic English reading and writing skills.
- Ability to advocate, with professionalism, for clients and family members for services and resources.
- Ability to work in an environment that is client-oriented, strength-based and values diversity.
- Ability to exercise appropriate judgment and decision-making.
- Ability to independently follow procedures and behave in a professional manner.
- Must have a valid California Driver’s License.
- Must pass DOJ’s background check.
Preferred Qualifications
- Bilingual/Bicultural in Cantonese, Vietnamese, or Hmong.
- Experience in community outreach and education.
- Experience facilitating group activities.

APCC offers two major programs including the Transcultural Wellness Center and Supporting Community Connections.
Transcultural Wellness Center (TWC) is an outpatient mental health service program designed to provide a full range of culturally and linguistically coordinated medical, therapeutic and support services for all ages including children, transitional age youth, adults, and older adults. The full-service partnership program is funded by the Sacramento County Division of Behavioral Health Services.
Supporting Community Connections is a mental health prevention and early intervention program that serves Cantonese, Hmong and Vietnamese speaking clients in the Sacramento area.

ESSENTIAL DUTIES
- This position is responsible for clerical and front office tasks including scheduling appointments, tracking referrals, maintaining client’s records including temperatures, generating reports, filing charts and paperwork, scanning and faxing client information, email and mail correspondence, greeting clients, answering phone calls and other related tasks, office and vehicle maintenance, cleanup and sanitizing of furniture, office space and vehicles, opening and closing of offices.
- Responsible for monitoring, tracking eligibility and compliance using an Electronic Health Record Database for clients.
- Oversee client record release, monitor billing, cancellations and claims to maximize reimbursement for services.
- Make necessary adjustments to billing/claims and monitor all correspondence regarding billing/claims.
- Work with IT staff to resolve computer or database issues, setup new laptops and other audio-visual equipment.
- Prepare brochures, handouts, business cards and banners for the agency.
- Maintain and update agency website and social media.
- Coordinate client activities or community education activities on site or in the community including setup and cleanup; must have ability to lift and move equipment, furniture weighing up to 20-30 pounds.
REQUIRED KNOWLEDGE AND SKILLS

ESSENTIAL DUTIES
- This position is a 100% time clinician position providing direct clinical services at the agency, clients’ home or in a community setting.
- Using a cultural formulation model, provide intake/assessment/diagnosis, service and discharge planning, and treatment for children, transition-age youth, adults, older adults, and families with mental health needs.
- Work with clients, collateral supports and Mental Health Counselors to develop culturally appropriate service and discharge plans. Coordinate care with Mental Health Counselors and Family and Peer Advocates, and with health, social services, and other community resources and supports.
- Provide in-home visitation services as well as transportation as necessary to conduct intakes/assessments/diagnosis, service and discharge planning, treatment and assist individuals and families with understanding the nature of mental illness and treatment benefits.
- Meet minimum billable minutes of 6,700 minutes per month to meet county requirements.
- Enter and complete required documentation in an electronic health record system, AVATAR within the standard 3 days of providing client care to meet county requirements.
- Provide individual, family and group psychotherapy.
- Facilitate psycho-social rehabilitation and support groups.
- Maintain a caseload consisting of persistently or severely mentally ill clients.
- Participate in Internal Utilization Review and Electronic Utilization Review.
- Provide after-hours coverage (24/7) crisis intervention on a rotation basis.
- Participate in community meetings as a representative of the agency. Act as a liaison for the agency to various other community agencies and organizations.
- Attend and participate in staff and service team meetings regularly.
- Perform other related duties as assigned.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
- Knowledge and experience of fundamental therapeutic models, corresponding interventions and treatment planning.
- Knowledge of clinical assessment and diagnostic skills to perform intake and case conceptualization.
- Knowledge of working with individuals with post-traumatic stress and refugee populations.
- Knowledge and awareness of the principles and methods of psycho-social rehabilitation practices that support wellness and recovery among children, transition-age youth, adults, older adults and families.
- Knowledge of and experience using the principles and methods of culturally competent assessment, diagnosis and treatment for children, transition-age youth, adults, older adults and families.
- Ability to exercise appropriate judgment and decision-making.
- Knowledge of and experience using the principles and techniques employed in community-based outreach for diverse communities.
- Knowledge and experience working with individuals who are gay, lesbian, bisexual, transgender or questioning.
- Knowledge and experience applying the methods and procedures for documentation and confidentiality.
- Ability to work in an environment that is client-oriented, strength-based and values diversity.
- Ability to develop and maintain effective working relationships with professional and paraprofessional staff, community partners and the public.
- Competent in the cultures and traditions of one or more of the ethnic groups served by the program.
- Competently bilingual in one or more of the languages served by the program, highly desirable.
- Strong interpersonal and communication skills.
MINIMUM QUALIFICATIONS
- Master’s Degree in Social Work or Marriage and Family Therapy and license-eligible as a mental health professional in the State of California by the Board of Behavioral Sciences.
- National Provider Identifier (NPI).
- Pass DOJ background check.
- One year of supervised, direct service work experience providing mental health services.
- Valid California Driver’s license and automobile liability insurance.
- Computer skills including Microsoft Word, Excel, Outlook and PowerPoint.
- Must be willing to work in the field and share weekend on-call responsibilities.
- Bilingual/bi-cultural preferred.
