TRANS CULTURAL WELLNESS (TWC)
POSITION: Mental Health Counselor
JOB CLASSIFICATION: Non-exempt
THE ORGANIZATION: Asian Pacific Community Counseling (APCC) promotes mental health and wellness in Asian and Pacific Islander (API) communities through culturally and linguistically relevant outreach, prevention, education, counseling and recovery support services and by providing training and consultation.
THE PROGRAM: Transcultural Wellness Center (TWC) is an outpatient mental health service program designed to provide a full range of culturally and linguistically coordinated medical, therapeutic and support services for all ages including children, transitional age youth, adults and older adults. The full service partnership program is funded by the Sacramento County Division of Behavioral Health.
The Mental Health Counselor is responsible for providing direct clinical services including case management, rehabilitation, service coordination and initial intakes to assess for presenting mental health symptoms and/or risk factors.

ESSENTIAL DUTIES
- Provide case management and service coordination to identify and develop community resources, assist the client to develop natural support systems for themselves and their families, make referrals, and assist with service linkage.
- Perform initial intakes and assess presenting mental health symptoms, current social and life problems and/or risk factors.
- Meet minimum of 6,700 billable minutes per month to meet productivity and billing target.
- Enter and complete required documentation in AVATAR within the standard 3 days of providing client care to meet county requirements.
- Communicate and collaborate with the client’s treatment team members including supervisors, clinicians, doctors, family advocate and peer partner to maintain good coordination of care for the client.
- Plan ahead, set priorities and schedule clients regularly to help clients reach their treatment goals.
- Plan and facilitate psychosocial rehabilitation activities and support groups to enhance the client’s ability to better function within their community and decrease isolation.
- Provide language translation/interpretation, cultural brokerage, advocacy for individuals and families to facilitate accurate understanding of client problems and appropriate service planning and access.
- Provide in-home services as needed to facilitate engagement, to help the client reach their treatment goals, to teach, train and develop functional and independent living skills, to promote resiliency, empowerment and rehab recovery.
- Outreach to the API communities to bring mental health awareness and to offer resources and assistance for available mental health services.
- Provide crisis intervention and after-hours assistant care as requested with clinician support.
- Provide suicide prevention support and safety plan when client is in crisis.
- Attend and participate in staff trainings and agency meetings, weekly supervision and team meetings as scheduled.
- Perform other related duties as assigned.
REQUIRED KNOWLEDGE AND SKILLS
- Ability to engage clients in rehabilitation recovery planning and activities.
- Ability to effectively conduct a risk assessment and safety planning if the client is in danger of harming themselves or others.
- Knowledge of or willingness to learn how best to provide employment and housing support services and housing placements.
- Ability to document appropriately and thoroughly and to maintain confidentiality.
- Ability to work in an environment that is client-oriented, strength-based and values diversity, culturally and spiritually.
- Ability to exercise appropriate judgment and decision-making.
- Strong interpersonal communication skills to maintain effective working relationships with staff and community partners.
- Ability to follow procedures and behave in a professional manner without constant supervision.
PREFERRED KNOWLEDGE AND SKILLS
- Knowledge of documentation requirement for the Division of Behavioral Health Services of Sacramento County.
- Knowledge and experience in providing mental health services to a culturally diverse community.
- Knowledge of or willingness to gain experience working with individuals who are gay, lesbian, bisexual, transgender or questioning.
- Knowledge or the ability to learn effective outreach techniques, resource development and community networking.
- Bilingual and bicultural in API languages/cultures a plus.
MINIMUM QUALIFICATIONS
- Bachelor Degree in Social Work, Psychology or in a related field preferred.
- 2 years of direct service experience in a mental health setting preferred.
- National Provider Identifier (NPI).
- Valid California Driver’s license and automobile liability insurance.
- Pass DOJ background check.
- Basic computer skills such as: WORD, EXCEL and email.

ESSENTIAL DUTIES
- This position is a 100% time clinician position providing direct clinical services at the agency, clients’ home or in a community setting.
- Using a cultural formulation model, provide intake/assessment/diagnosis, service and discharge planning, and treatment for children, transition-age youth, adults, older adults, and families with mental health needs.
- Work with clients, collateral supports and Mental Health Counselors to develop culturally appropriate service and discharge plans. Coordinate care with Mental Health Counselors and Family and Peer Advocates, and with health, social services, and other community resources and supports.
- Provide in-home visitation services as well as transportation as necessary to conduct intakes/assessments/diagnosis, service and discharge planning, treatment and assist individuals and families with understanding the nature of mental illness and treatment benefits.
- Meet minimum billable minutes of 6,700 minutes per month to meet county requirements.
- Enter and complete required documentation in an electronic health record system, AVATAR within the standard 3 days of providing client care to meet county requirements.
- Provide individual, family and group psychotherapy.
- Facilitate psycho-social rehabilitation and support groups.
- Maintain a caseload consisting of persistently or severely mentally ill clients.
- Participate in Internal Utilization Review and Electronic Utilization Review.
- Provide after-hours coverage (24/7) crisis intervention on a rotation basis.
- Participate in community meetings as a representative of the agency. Act as a liaison for the agency to various other community agencies and organizations.
- Attend and participate in staff and service team meetings regularly.
- Perform other related duties as assigned.
REQUIRED KNOWLEDGE, SKILLS AND ABILITIES
- Knowledge and experience of fundamental therapeutic models, corresponding interventions and treatment planning.
- Knowledge of clinical assessment and diagnostic skills to perform intake and case conceptualization.
- Knowledge of working with individuals with post-traumatic stress and refugee populations.
- Knowledge and awareness of the principles and methods of psycho-social rehabilitation practices that support wellness and recovery among children, transition-age youth, adults, older adults and families.
- Knowledge of and experience using the principles and methods of culturally competent assessment, diagnosis and treatment for children, transition-age youth, adults, older adults and families.
- Ability to exercise appropriate judgment and decision-making.
- Knowledge of and experience using the principles and techniques employed in community-based outreach for diverse communities.
- Knowledge and experience working with individuals who are gay, lesbian, bisexual, transgender or questioning.
- Knowledge and experience applying the methods and procedures for documentation and confidentiality.
- Ability to work in an environment that is client-oriented, strength-based and values diversity.
- Ability to develop and maintain effective working relationships with professional and paraprofessional staff, community partners and the public.
- Competent in the cultures and traditions of one or more of the ethnic groups served by the program.
- Competently bilingual in one or more of the languages served by the program, highly desirable.
- Strong interpersonal and communication skills.
MINIMUM QUALIFICATIONS
- Master’s Degree in Social Work or Marriage and Family Therapy and license-eligible as a mental health professional in the State of California by the Board of Behavioral Sciences.
- National Provider Identifier (NPI).
- Pass DOJ background check.
- One year of supervised, direct service work experience providing mental health services.
- Valid California Driver’s license and automobile liability insurance.
- Computer skills including Microsoft Word, Excel, Outlook and PowerPoint.
- Must be willing to work in the field and share weekend on-call responsibilities.
- Bilingual/bi-cultural preferred.
